Epileptogenic networks in extra temporal lobe epilepsy

چکیده

Abstract Extra temporal lobe epilepsy (eTLE) may involve heterogenous widespread cerebral networks. We investigated the structural network of an eTLE cohort, at postulated epileptogenic zone later surgically removed, as a node; resection (RZ). hypothesised patients with abnormal connection to/from RZ, to have proportionally increased abnormalities based on topological proximity in addition poorer post-operative seizure outcome. Structural and diffusion MRI were collected for 22 pre- post-surgery, 29 healthy controls. The connectivity RZ prior surgery, measured via generalised fractional anisotropy (gFA), was compared Abnormal connections identified those substantially reduced gFA (z < −1.96). For one or more closer distance had higher proportion abnormalities. minority seizure-free (3/11) connections, whilst most non-seizure free (8/11) RZ. Our data suggest proportional associated chance freedom post-surgery.

Comparison of temporal lobe epilepsy with hippocampal sclerosis and temporal lobe epilepsies due to other etiologies

Background: This study compares the clinical characteristics of patients with mesial temporal lobe epilepsy with hippocampal sclerosis (mTLE-HS) with those who have temporal lobe epilepsy (TLE) due to other etiologies.   Methods : In this retrospective study all patients with a clinical diagnosis of TLE were recruited in a referral outpatient epilepsy clinic at Shiraz University of Medical Sc...

Endoscopic Transsphenoidal Resection of a Medial Temporal Arachnoid Cyst in a Patient with Temporal Lobe Epilepsy

Abstract Introduction: Arachnoid cysts are developmental cystic lesions which may be found as an incidental finding on neuroimaging or present with symptoms of headache, seizure and neurologic deficit. Presentation with seizure is more common with larger sizes and temporal location. Presentation with TLE is rare, and fenestration of cysts had variable results for seizure control. Here in, we r...

Localization of epileptogenic zone in temporal lobe epilepsy by ictal scalp EEG

Our aim was to evaluate the ability to localize the epileptogenic zone in temporal lobe epilepsy (TLE) by ictal scalp electroencephalogram (EEG). Using simultaneous video recording, we analysed scalp EEG activity during ictal periods in 38 patients (30 patients with medial TLE (MTLE) and eight with lateral TLE (LTLE)). In 14 patients, intracranial ictal EEGs were recorded with depth electrodes,...

Ictal Depth EEG and MRI Structural Evidence for Two Different Epileptogenic Networks in Mesial Temporal Lobe Epilepsy

Hypersynchronous (HYP) and low voltage fast (LVF) activity are two separate ictal depth EEG onsets patterns often recorded in presurgical patients with MTLE. Evidence suggests the mechanisms generating HYP and LVF onset seizures are distinct, including differential involvement of hippocampal and extra-hippocampal sites. Yet the extent of extra-hippocampal structural alterations, which could sup...
